October 18, 2024 - Early Childhood Systems 101 

Please join us for a conversation on how the medical home and Vermont’s early childhood system can partner more closely and effectively in service of children and families. We will (re)introduce you to key partners and programs in the early childhood system: Parent Child Centers, Children’s Integrated Services (CIS), Help Me Grow (HMG) Vermont, and more, as well as share examples of successful integrative models, including DULCE. We aim to have a dynamic dialogue that identifies opportunities for medical homes to leverage early childhood experts in their region for team-based care.  

 

November 7, 2024: Family-Engaged Developmental Monitoring 

Please join us for a shared exploration of Vermont Developmental Surveillance, Screening and Referral approach, including tools, online screening options and partnerships between medical home, Help Me Grow and Children’s Integrated Services (CIS). 

We are planning an interactive session with your observations, questions and challenges with current implementation of these important screens. 

Developmental and behavioral screening is a whole-population strategy to improve child health outcomes. Screening, together with routine developmental surveillance, provides an opportunity to monitor if a child is learning age-appropriate skills and to identify any developmental or behavioral concerns as soon as possible, enabling intervention during the most critical periods of development. When young children are not screened systematically, opportunities for early identification, intervention, and treatment may be delayed. Early identification of children at risk for developmental delays is an integral function of the primary care medical home.

Touchpoints Training 

Blueprint and the Vermont Department of Health, Division of Family and Child Health are offering a Touchpoints training intended for a select group of family and child specialists and pediatric professionals. There are only 20 seats for the series, which will take place over seven weeks in January and February of 2025, starting January 8 and repeating every Wednesday until February 12. The virtual components of the series will take place over Zoom from noon until 1:30. Please contact your Program Manager for more information.
