CMS Multi-Payer Advanced Primary Care Practice Demonstration Reports
From 2011 through the end of 2016, Vermont participated in the Centers for Medicare & Medicaid Services (CMS) demonstration of multi-payer reform initiatives aimed at making advanced primary care practices more broadly available.
