Food Access and Health Care Integration, A Look at Three Promising Practice Models in VT

Objectives: Join us for a small taste of three evidence-based or promising practice models that support food access and health care integration. On the menu, Lamoille Health Partners, Little Rivers Health Care and Northern Tier Center for Health (NOTCH) will highlight their programs that are unique to the regions they serve. We’ll dip into Medically Tailored Meals, Produce Prescriptions, and the Social Grocery Store initiative and how these programs are helping patients with diabetes, cardiovascular disease, and other chronic health conditions. Participants will learn how each program began, is sustained, and the challenges and successes experienced over the past three years. You may just leave hungry for more!

Connecting with Your Community through Research: An Exploration of NNE CO-OP PCBRN and the Card Study Methodology

Objectives:  Join us for an enlightening session as we delve into the Northern New England CO-OP Practice and Community Based Research Network (NNE CO-OP PCBRN). This voluntary and cooperative network comprises primary care clinicians across New Hampshire, Vermont, and Maine. Discover opportunities to enhance your research skills and engage in studies collaboratively with patients, academic researchers, and local communities. As the longest continuously operating practice-based research network (PBRN) in the country, we take pride in our history of enhancing patient outcomes, prioritizing patient values, and developing effective methodologies for busy community-practice settings.

In this 1-hour workshop, familiarize yourself with your local research network and explore ways to actively participate. Our presentation features a nationally recognized expert introducing the card study methodology—a widely embraced study design in PBRNs known for its low clinical burden, high response rates, and straightforward implementation.

The objectives of this presentation are:

  1. Explain the concept of a Practice-Based Research Network (PBRN)
  2. Identify opportunities to get involved with your local PBRN i.e., the NNE CO-OP PCBRN
  3. Identify 2-3 common study design methods for ambulatory clinical research
  4. Describe the components of a Card Study research project.
