New profiles available now

In November 2019 the Blueprint sent its latest patient-centered medical home data profiles to participating practices. If you are a participating practice with more than 300 attributed practices, you should have received a profile. If you haven't, please contact any member of the Blueprint team.

New profiles offer quicker read on successes and opportunities

The Blueprint has produced patient-centered medical home data profiles for many years, as part of our commitment to offering data and insights to all of our programs, with the goals of helping participants understand their performance and informing continuous quality improvement. This year, the Blueprint is trying a new approach — reporting on measures we know are important to patient-centered medical homes and their patients in a simplified, dashboard style. Our goal was to create a profile everybody in a practice can quickly scan to know how the practice is doing compared to other practices around the state and compared to the practice's past performance. Thank you to all the practices, Program Managers, and Quality Improvement Facilitators who participated in the research, design, and prototyping of the new profiles!

Sample profile

Profiles with real data are shared privately with practices. This sample profile for "Acme Primary Care" is available to anyone who wishes to review the profile design or see which measures are included.
