Blueprint Community Health Profile Data By Hospital Service Area (HSA)
The Blueprint supports data-driven population health improvement by producing annual profile data on the health status, health care utilization, and health care outcomes of the patients of each Blueprint hospital service area community.
Community health profile data are used by Community Collaboratives and other local workgroups. These data can inform and complement Community Health Needs Assessments and other community data products.
Recent community profile datasets are available here, viewable through linked Excel pivot tables and pivot charts (use the filters to select the measures and population breakouts you wish to see displayed):
Data Through Calendar Year 2022. (Most recent profile data available, as of 2024.)
Data Through Calendar Year 2021.
Data Through Calendar Year 2020.
Data Through Calendar Year 2019.
(Please note that null results for particular measures may be a consequence of blinding (suppression), to protect confidentiality for cells representing information for less than 11 individuals.)
Risk-adjusted payment measures are available here.

Whole Population Data Source
Community health profile measures are built from health insurance claims from all public insurers and Vermont's major commercial insurers. Most practices and communities design interventions and improvements that support all their constituents, regardless of insurer. Yet whole population data can be difficult to find. The Blueprint community profiles are a rare whole-population data source, designed to complement the way practices and community coalitions work.
Combining Claims and Clinical Data for New Insights
In addition to the claims data, the Blueprint Profiles use the clinical data that flows from practice and hospital electronic medical records through the Vermont Health Information Exchange (HIE) into the Vermont clinical registry. This clinical data is combined with claims data to create unique views of local health care interventions for key chronic conditions - for instance, the proportion of individuals diagnosed with diabetes whose last hemoglobin A1c test was in poor control.
Full Methodology for the Blueprint Profiles
Detailed methodology for every measurement presented in the Blueprint profiles can be found on the Community Health Profiles Methodology and Supporting Documentation page. The methodology may change slightly for each set of profiles.
Blueprint Community Health Profiles are organized by Vermont Hospital Service Area (a slightly different designation than the Blueprint Health Service Areas, which dictate the coverage of Blueprint Community Health Teams). The towns included in each Hospital Service Area are mapped by the Vermont Department of Health here and the practices in each Hospital Service Area are listed in the Supporting Documentation for the Blueprint Profiles. Historical Community Health Profiles are available on the Hospital Service Area pages.
