Community Health Profiles

Blueprint Practice Profiles and Blueprint Community Health Profiles

The Blueprint supports data-driven population health improvement by producing bi-annual profiles of the health status, health care utilization, and health care outcomes of the patients of each Blueprint practice and community.

Practice Health Profiles help practices identify ways they can better serve their patients, and track the success of quality improvement initiatives.

Community Health Profiles are used by Community Collaboratives and other local workgroups. These profiles can inform and complement Community Health Needs Assessments and other community data products. The Blueprint's Community Health Profiles are organized by Hospital Service Area, with links below:

Whole Population Data Source

Both the practice and community profiles are built from health insurance claims from all public insurerers and Vermont's major commerical insurers. Most practices and communities design interventions and improvements that support all their constituents, regardless of insurer. Yet whole population data can be difficult to find. The Blueprint Profiles are a rare whole-population data source, designed to complement the way practices and community coalitions work.

Combining Claims and Clinical Data for New Insights

In addition to the claims data, the Blueprint Profiles use the clinical data that flows from practice and hospital electronic medical records into the Blueprint Clinical Registry. This data is combined with claims data to create unique views of local health care interventions for key chronic conditions - for instance, the proportion of individuals diagnosed with diabetes whose last hemoglobin A1c test was in poor control.

Accessing New and Historical Practice Health Profiles and Community Health Profiles

Practice Health Profiles are provided directly to Blueprint Patient Centered Medical Homes by email. Practice Managers who need their profile re-sent may contact the Blueprint Data Analyst.

Blueprint Community Health Profiles are organized by Vermont Hospital Service Area (a slightly different designation that the Blueprint Health Service Areas, which dictate the coverage of Blueprint Community Health Teams). The towns included in each Hospital Service Area are mapped by the Vermont Department of Health here and the practices in each Hospital Service Area are listed in the Supporting Documentation for the Blueprint Profiles. All current and historical Community Health Profiles are available on the Hospital Service Area pages.
